Set Cover Photo on Create Image page

Out of the box an item is created and saved THEN the option to set cover photo becomes available. We'd like to be able to set the cover photo on the upload of the file (checkbox) or as a separate button on Create Item. Wondering if anyone has done this and would be willing to share?

I believe the first photo you upload will be your cover... So be sure to upload that one first.

You'd be looking at quiet a bit of programming (advanced programming at that) to pull that off. I looked into it several years ago and the changes that I'd have to make to the entire system were way to much for me to consider it.
