Prefix Permissions

I seem to be having issues with prefixes disappearing after users update content (either tabular or sidebar custom fields)

After they update and save the item the prefix is reset to nothing. I have had a look around the perms and they all look fine.

Scratching my head a little on this one. Any ideas Bob?

There is nothing tied to permissions for editing the prefix... ie, if you can edit the item, you can set/remove/edit the prefix. Have you made any modifications to the item edit template at all? Can you reproduce this yourself?
